You're underestimating just how sheltered people can be in this world 😆 There's a ton of medical case studies of couples coming into fertility clinics asking why they aren't pregnant, just for the clinician to find out that they have no clue what sex is (they thought sleeping together on a bed and holding hands was all they needed to do)... The doctor I worked for essentially told me each fertility doctor goes through a series of sex-related questions before even thinking about fertility issues/concerns 😆
There was a teenager + mother I saw the other day who essentially asked us to not ask any questions related to boyfriends/sex/drugs because she didn't want her daughter knowing any of that... personally speaking, this just leads to a lot of problems since shows don't do a good job teaching this
