I don’t know the full context of that ladie’s speech .. but imo what she tried to say was there is many people out there who couldn’t lose weight due to some biological and hereditary conditions despite of their constant effort ..those people also may get hurt or feel inferior by his reckless comment of calling them “ disgusting “...
But in his video he clearly says that there might be individuals who have some harmonal problems like thyroid/ genetic reasons but for most of them it's sheer laziness and unhealthy food habits. Still most people keep coming with some lame excuses rather than admitting this. So that's why he needed to be harsh, that too with U.S being ranked as the most obese nation
Also you need to consider where he is coming from - he is a drill sergeant who trains soldiers regularly. So fitness is a matter of life/death situation there
