This clears that they r using kinnar as a umbrella term for saumya's medical condition.

She is a girl just with disorder with her sexual organs that's it

She is right now not suitable for physical relationship or cannot conceive. But such conditions do have cures n it's not a sex change operation. Atleast if not capable of being a mother saumya can get capable to live a married life

I think this clears many doubts
